My jacket is also rarely accepted, even by my girlfriend. I guess they're trying to be just as lady-like, by making sure you're also warm?
But I always hold doors open for females, offer my jacket, walk on the street side, etc. Also, I'm all about this small detail: The holding of the lower back. Or leading by lightly pushing the lower back. I don't know what it is, but it's less aggressive than the full back, and less sleezy than the ass.
Manners?
For a lady I will always get the door, this goes along with giving them your jacket on a cold night. And even simple things like walking on the street side of the sidewalk. These are just basic manners. How can you consider yourself a Gentleman with out manners. Some females are not ladies and no I will not get the door for them
Manners!
Manners are really important, and I think having proper etiquette is one of the most important parts of being a classy fellow. But manners and etiquette aren't about knowing all the minute details of which fork to use when eating salad or whatnot. I think true etiquette is making those around you as comfortable as possible. For some people that might mean relaxing what you normally do so they don't feel out of place.
When it comes to your specific example I always try and open the door for ladies, and I do try and offer my jacket (although it's rarely accepted) on cold nights.
